FOR FLORIDA HEALTHCARE LICENSING
DOH Background Screening for Medical Professionals
As of July 1, 2025, most Florida health care practitioners must complete Level 2 background screening requirements when applying for initial licensure or renewing an existing license.
5 Star Mobile Fingerprinting provides electronic LiveScan fingerprint capture in Orlando for applicable Florida Department of Health screening requirements.
Important: Your ORI depends on your profession. Bring the profession-specific ORI shown in your DOH screening instructions. An incorrect ORI can prevent the board office from receiving your fingerprint results.
Common DOH Professions
- RN, LPN & APRN
- Medical Doctors
- Physician Assistants
- Certified Nursing Assistants
- Dental professionals
- Mental health professionals
- Physical & occupational therapy
- Other applicable DOH-regulated professions
YOUR PROFESSION MATTERS
DOH Does Not Use One ORI for Every Practitioner
The Florida Department of Health assigns profession-specific ORI numbers so screening results are routed correctly. Do not use an ORI simply because another healthcare worker used it.
Bring your DOH instructions or Livescan information so the ORI for your profession can be confirmed before fingerprinting.
Common DOH ORI Examples
Nurses — RN, LPN, APRN: EDOH4420Z
Medical Doctor: EDOH2014Z
Physician Assistant: EDOH4700Z
Certified Nursing Assistant: EDOH0380Z
Dental Professions: EDOH4560Z
Mental Health Professions: EDOH4550Z
Physical Therapy: EDOH4690Z
Always follow the current ORI listed for your profession by Florida DOH.
START YOUR SCREENING
Use CHAI and Your Profession-Specific ORI
Florida DOH directs applicants to initiate applicable screenings through the Clearinghouse Applicant Initiated (CHAI) system, create their profile, select the appropriate screening, and then schedule with a Livescan provider.
Bring your profession-specific ORI to the fingerprinting appointment.

INITIAL LICENSURE
Applying for a Florida Healthcare License?
Applicants in screened professions must complete the required Level 2 background screening as part of the licensing process. Start the applicable screening and use the ORI assigned to your profession.
LICENSE RENEWAL
Renewing an Existing DOH License?
Practitioners in screened professions who have not already completed a qualifying Level 2 screening may need to satisfy the requirement before the next applicable renewal.
Eligible fingerprints may be retained in the Clearinghouse for up to five years, so do not assume you need a new fingerprint appointment at every renewal. Check your screening status first.
Not every healthcare credential follows the same rule. Florida DOH currently lists Emergency Medical Technicians, Paramedics, Pharmacy Interns, Registered Pharmacy Technicians, and Radiologic Technologists as exempt from the general background-screening requirement unless applying through the military active-duty spouse licensure pathway. Follow the instructions for your specific profession.
